Abstract

This is a prospective study envolving multiple pregnancies delivering at Clínica Obstetrica do Hospital das Clínicas da Faculdade de Medicina da USP. The aim of the study is to evaluate and compared immunoglobulin concentrations in maternal serum and umbilical cord after birth. Expression of specific immunoglobulins receptors in the placenta will be examined. Antibodies for specific neonatal infections (IgA anti-Stretococcus, anti-LPS Klebsyella and Pseudomonas) will also be evaluated in the differents compartments (maternal and umbilical cord). The present study is based on the hypothesis that in multiple pregnancies, maternal to fetal transplacental antibodies passage is different from singleton pregnancies. This difference is mainly assigned to the concurrent development of two or more fetuses in the uterus, with distinct total placental mass from those of single pregnancies. These differences induce adaptative needs in the maternal body that may interfere in the dinamic placental antibody transfer during pregnancy. With the intention to reduce the perinatal morbidity and mortality rates related to infections complications in the first days of life, the knowledgement of the factors that interferes in the placental transfer of antibodies in twin pregnancies constitute an important element to adequate the clinical management of these pregnancies. The Hospital das Clínicas da Faculdade de Medicina da USP is a tertiary referral center for obstetrics and perinatal care with high number of multiple pregnancies cases. Therefore it is a propitious place to develop this study to investigate the formulated hypothesis. (AU)
