Abstract

The search for inflammatory markers in the diagnosis of Ventilator-Associated Pneumonia (VAP) have increased over the last years. The objectives of this clinical study are: 1) To investigate and monitor the inflammatory profile in the lungs, blood circulation and oral cavity in patients from Intensive care unit patients (ICUP) requiring mechanical respiratory ventilation, by the levels of - IL-1±, IL-1², IL-6, IL-8, IL-10, TNF-± - and their antagonists - IL1-RA, IL-6 RA, sTNF- RI, sTNF-RII; 2) To investigate and monitor the inflammatory profile in the lungs, blood circulation and oral cavity in patients from ICUP requiring mechanical respiratory ventilation, by the levels of - MMP-1, MMP-2, MMP-8, MMP-9 - and their antagonists - TIMP-1, TIMP-2; 3) To investigate and monitor the levels of endotoxins profile in the lungs, blood circulation and oral cavity in patients from ICUP requiring mechanical respiratory ventilation by the LAL-assay (Limulus Amebocyte Lysate); 4) To investigate the biomarkers sTREM-1, prolactin, C-reactive protein in plasma samples in patients from ICUP requiring mechanical respiratory ventilation. Samples will be collected from the bronchoal-alveolar lavage fluid by an oral tracheal suction catheter, the blood circulation by using sterile "vacutainer" tubes containing heparin and the oral cavity (Crevicular fluid) using 3 sterile/ apyrogenic paper points, of 15 patients ICUP requiring mechanical respiratory ventilation. The levels of selected biomarkers and their antagonists will be determined by ELISA assay (Enzyme-Linked Immunosorbent Assay). The levels of endotoxins will be determined by the KQCL-test (LAL-assay). Data will be typed on a spreadsheet and statistical analyzed by SPSS for Windows and package Statistica 9.0. (AU)
