Experimental evaluation of platelet-rich plasma in the treatment of bisphosphonate-related osteonecrosis of the jaw

Abstract

Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is an oral complication in which clinical and experimental studies are required to elucidate its pathogenesis and treatment, although there are much scientific knowledge since its first report. There is no consensus regarding the best therapeutic approach of BRONJ showing satisfactory and predictable outcomes. The use of bone resection combined with platelet-rich plasma (PRP) is an alternative treatment modality which has shown satisfactory and promising outcomes in symptomatic cases. The aim of this study is to evaluate this alternative treatment in an experimental rat model. 32 rats will receive intravenous administration of zoledronic acid for induction of BRONJ. After the extraction of the right upper incisor and diagnosis of BRONJ, surgical treatment will be performed. In the experimental group, marginal resection of osteonecrosis will be performed followed by use of PRP. The control group will undergo the same procedures described for the experimental group, without PRP. After 14, 28 and 42 days, the animals will be euthanized and the evaluation will be done by quantitative analysis of new bone formation by computed microtomography. Moreover, morphological, morphometric and immunohistochemical microscopic analyses using VEGF and CD31 antibodies will be performed in order to assess microvascular density in the healing of the specimens.
