Use of PTT (Pulse Transit Time) for non invasive blood pressure measurement in veterinary applications

Abstract

The current most used method of noninvasive blood pressure measurement is from the end of 19th century, developed by Scipione Riva-Rocci in 1896. In this exam, a cuff is inflated and deflated in the patient's arm to identify indirectly systolic, diastolic and mean blood pressure. It became popular because it is easy, quick and cheap procedure and can be done with a stethoscope, doppler or oscilometric sensor, the last one the only automated method.Oscilometric method have additional inconvenient. For human medicine, the patient must keep still and seated. If a constant monitoring is necessary, as in ABPM (Ambulatory Blood Pressure Monitoring), measurement is not accurate due to patient's movement and, once it uses air pump to inflate the cuff, disturb patient's sleep. For veterinary medicine, it is very difficult to use oscilometric method in small awaken animals due to movement. Success rate is extremely low and measurement is not accurate, leading to a procedure where 5 measurements are necessary to mitigate the lack of accuracy. Fr several applications, automated blood pressure measurement is necessary. As examples, we have ABPM and surgeries - human and veterinary - where constant measurement is necessary. The use of a noninvasive, automated, cheap and easy method that suffered little influence from external artifacts would bring important benefits to the procedures that relies on blood pressure measurement.Blood pressure measurement that uses PTT (Pulse Transit Time) is based on the time that the pulse wave takes to travel from the heart to the edge of the body. The delay between proximal and distal waveforms has a direct correlation with blood pressure. The measurement of this delay can potentially allow truly continuous noninvasive blood pressure measurement. Once there is no need to use a cuff, this method can be efficiently used in applications where is not possible to keep the patient still. This research project propose the use of blood pressure measurement based on PTT for veterinary application. The continuous measurement using this method can replace invasive measurement with advantages concerning measurement quality and patient welfare. The basics of this method relies on the modeling of arterial walls and arterial wave propagation. In this project, these mathematical models will be developed and used as basis for an algorithm using digital signal processing techniques. The result can be used in a wearable device for animals with different sizes, allowing for the first time a continuous veterinary blood pressure measurement. The project is divided in 3 parts: analysis of current scenario (state of art and market), development of a competitive advantage and validation of this advantage. Canvas business model will be used in all project parts to drive the strategic decisions. (AU)
