Abstract

Effect of Lactose Free Diet on Patients with Irritable Bowel Syndrome the Irritable Bowel Syndrome (IBS) is a disease of the gastrointestinal tract that affects around 20% of the adult population, presently defined by the called Roma III criterion. Some symptoms, when they are present, reinforce the diagnosis of the IBS such as evacuation urgency (mainly postprandial), feeling of incomplete bowel movement, presence of mucous at the stool and e distension or abdominal bloating. Approximately 60% of the patients with IBS refer that some kinds of food worsen their symptoms. Therefore one of the several food components that is most referred by the patients with IBS as an aggravating of the symptoms is the lactose. After a careful historical and a review at the food diary if there is any sign of bad absorption of lactose the exam of respiratory hydrogen must be considered. In the presence of these controversies we have thought that it is worthy the study of the effects of a lactose free diet for the patients with IBS with or without a positive test for bad absorption of lactose (respiratory test). The results of such research will contribute for the enlightening of the necessity or not of the routine of research on the bad absorption of lactose in patients with IBS and the efficacy of the lactose free diet for these patients independently of the results of the test of lactose absorption. The patients will be distributed in two groups, group A with positive hydrogen test for lactose (that indicates bad absorption of lactose) and group B with negative test of respiratory hydrogen for lactose (normal test, with no bad absorption of lactose). During the respiratory test the symptoms that are triggered by the ingestion of lactose will be noted, if it is the case. Each patient will have to follow the lactose free diet for a period of 4 weeks. The procedures that will be carried out: Dietary Inquest with a Feeding Record of 24 hours (R24h); the application of the lactose free diet; Anamneses and clinical and surgical history; general physical examination, vital signs and assessment of clinical symptoms; Evaluation of the Intensity of the Signs and Symptoms of IBS; Analogical Visual Scale with the Intensity of signs and symptoms; Evaluation of the form and consistency of the stool; Evaluation of the Intensity of the Symptoms related to the IBS; Global Assessment of the Treatment by the Searcher; Global Assessment of the Treatment by the Patient.
