Abstract

Pancreas Transplantation (PT) has become a therapeutic alternative to promote normoglycemia in Diabetes Mellitus type 1 (DM1) high-risk patients while protecting against secondary complications, but with the risk of adverse outcomes. Historically, the fear of early graft loss leads the selection to be extremely conservative in choosing organs for transplantation. Yet the number of organs offered did not grow in proportion to its needs, leading transplant services to seek organ donors not considered ideal: they are within the criteria for a donation, but neighboring one or more features, and it can bring repercussions on function and graft survival. Appropriate selection of grafts is a key component to the success of the procedure. Faced with similar concerns, Feng et al (2006) proposed the Donor Risk Index (IRD) which relates the characteristics of the donor with the recipient's characteristics, trying to predict the risk of adverse outcomes in liver transplantation. Because of its usefulness, Axelrod et al (2010) adapted the IRD for pancreas transplantation. Objectives: 1 - check the usefulness of IRD in the PT to relate characteristics of the donor and receptor evolution in Brazil. 2 - Check the potential of IRD in PT in predicting graft survival. Method: retrospective analysis of 200 medical records of the Grupo Hepato/Hospital Beneficencia Portuguesa applying IRD and of the evolution of organ and patient. Exclusion criteria: incomplete medical records and patients younger than 18 years. Determination of sensitivity/specificity of the test. Application of other statistical tools: Logistic Regression, Survival Tablets/Kaplan-Meier, Pearson correlation, ROC curve.(AU)
