Analysis of the functional aspects of sperm and testicular oxidative stress in subjects undergoing metabolic surgery

Abstract

Introduction: Nowadays, obesity stands out as a major cause of male infertility. Excess body fat is related to changes in the profile of male sex hormones and to the increase in scrotal temperature, resulting in changes in both semen quality and sperm function. Metabolic Surgery (Gastric Bypass Roux-Y) is an approach indicated and performed successfully for the treatment of obesity, aimed at improving the quality of life of these patients. Objectives: To evaluate the effects of weight loss after metabolic surgery in semen analysis and functional aspects of sperm (DNA integrity and mitochondrial activity). The level of lipid peroxidation will be measured in the seminal plasma of these patients in order to investigate the relationship between the degree of obesity, oxidative stress and functional changes in sperm. In addition, there will be a quantification of the total antioxidant capacity of the samples and an analysis of the hormonal profile of each individual. Methods: The study will include the evaluation of 20 patients treated at the Surgical Gastroenterology at tha Hospital São Paulo and with indication of metabolic surgery (BMI e 40,0kg/m2). The blood (for hormone dosage) and semen samples of patients will be collected before and after surgery (3 in 3 months) for a total of 12 months. The semen samples will be analyzed according to the parameters of the WHO (2010) and morphology according to strict criteria of Kruger (1986). The integrity of sperm nuclear DNA will be evaluated by alkaline comet assay, in which the cells are classified according to the intensity of DNA damage of grade I (high DNA integrity) to grade IV (highly fragmented DNA). The mitochondrial activity will be evaluated by a colorimetric method, where the cells are classified into class I (100% of active mitochondria) to IV (100% of inactive mitochondria). The level of lipid peroxidation will be determined using a method based on the reaction of MDA with TBA. For the quantification of the total antioxidant capacity of samples will be utilized a commercial kit (QuantiChrom Antioxidant Assay Kit).Keywords: Male infertility, semen, obesity, oxidative stress, DNA fragmentation, mitochondria/metabolism.
