Abstract

Redistribution of blood flow in the fetal circulation is an important phenomenon of obstetric physiology. Studies in humans have shown that the redistribution of fetal blood flow is not limited to pathological conditions can also occur due to changes of maternal positioning. Objectives: To evaluate the effects of maternal supine position in the fetal circulation by Doppler examination, and comparing indices in the fetal middle cerebral artery, uterine artery and umbilical vein in the change of left lateral decubitus position for the mother to supine position, in term pregnancies. Methods: 20 pregnant women who meet the following inclusion criteria will be included: pregnancy with single living fetus; age over 18 years; absence of comorbidities or pregnancy complications; gestational age between 36 and 40 weeks. Exclusion criteria are: newborns with congenital defects or other defects not previously detected; newborn of a weight not appropriate for gestational age, not diagnosed in antenatal period. The research will consist on the examination of Doppler parameters of the fetal circulation at rest in the maternal left lateral decubitus position for 10 minutes and then supine with the dorsum elevated at 5 minutes and 10 minutes. The following vessels will be assessed: umbilical artery, fetal middle cerebral artery and umbilical vein; by pulsatility index, systolic diastolic ratio, maximum systolic velocity in the arteries; and blood flow volume in the umbilical vein. Results will be compared by the Student t test or Wilcoxon test for paired samples, verifying the adaptation of the fetal circulation opposite change of maternal position.
