Impact of asymptomatic deep venous thrombosis in the development of post-Purpura syndrome

Abstract

Deep Venous Thrombosis (DVT) constitutes a disease which the main features are the occurrence of thrombus into the venous blood system, leading to vessels’ lumen obstruction, mainly affecting the lower limbs. Diagnosis is made by applying accessories imaging tools in patients presenting suggestive clinical signs and symptoms, however, in their absence, there is the occurrence of asymptomatic Deep Vein Thrombosis (aDVT), which can develop to Pulmonary Emboli and Post-Thrombotic Syndrome (PTS). Still, there is a lack of data in the literature showing the progression of the ADVT into these other events. Objectives: To determine the prevalence of SPT in a population of patients with confirmed history of aDVT, to compare the morbidity of the SPT event in a population of patients with a history of aDVT with the SPT event in a population of patients with previous sDVT, to compare PTS clinical repercussion and ultrasonography between sDVT and aDVT, compare initial ultrasonographic data (first DVT episode) with current ultrasonographic data and correlate with the clinical severity of SPT. Methods: 579 Inpatients and outpatients were investigated in order to confirm the presence of DVT in the symptomatic limb. Contralateral limb, if asymptomatic, was also investigated systematically in the attempt to diagnose aDVT, which leaded to 108 patients (18,65%) at a previous study. Thus, aDVT and symptomatic DVT patients will be invited to show up in the Laboratório Vascular (Hospital das Clínicas - Faculdade de Medicina de Botucatu/UNESP), in order to be submitted to a new clinical and ultrasonografic evaluation. The proportion will be 1:2 (aDVT:sDVT) and the clinical analysis will consist on the application of Villata score concerning PTS diagnosis and ultrasoonographic findings. Added to that, the analogically visual scale will be applied in order to determine pain intensity. Key Words: Deep Venous Thrombosis; Venous Insufficiency; Post-Thrombotic Syndrome (AU)
