Abstract

Context: Frailty is a clinical syndrome associated with negative health outcomes. The study of frailty among Brazilian elderly is of particular relevance, as they live in different conditions compared to European and North American elderly, in terms of income, education, access to health and social opportunities. These differences can change the profile of this syndrome in Brazil, as well as its antecedents and outcomes. Objectives: This investigation aims to carry out the follow-up assessment after seven years of two cohorts of seniors who were enrolled in the population-based study FIBRA coordinated by UNICAMP with data collection conducted between 2008 and 2009 in Campinas (n = 900) and Ermelino Matarazzo ( n = 384). The following research questions should be answered: 1. Among the robust seniors in 2008/2009, how many have become pre-frail and frail and which factors, among a set of sociodemographic, psychological, social and health parameters, predict frailty?; 2. The pre-frailty and frailty identified in 2008/2009, in a portion of the original sample, are associated with which health outcomes after seven years (e.g., death, immobility, functional dependency, dementia, hospitalization)? 3. Among the robust, pre-frail and frail participants in 2008/2009, how many have developed major or mild neurocognitive disorder and which factors, among a set of sociodemographic, psychological, social and health parameters predict these conditions? Methods: The participants of the original sample will be contacted by letter and then by telephone to identify the participant's current status and to schedule a home interview. The original protocol of the FIBRA study, after minor adjustments, will be applied to collect information about demographics, health, neurocognitive and psychiatric profile, anthropometry, frailty makers, psychosocial characteristics and nutritional intake. The family of the participant will be interviewed to obtain information regarding the neurocognitive profile and should be the main informant if the participant is unable to participate. This longitudinal study will generate information on factors that predict long-term frailty among the robust elderly in the baseline and the outcomes of frailty among the elderly initially identified as frail or pre-frail. This investigation should provide information about factors that might help to delay or mitigate the negative health outcomes associated with frailty.
