Epidemiologic analysis of acute kidney injury in a tertiary hospital: a retrospective cohort study in the Clínicas Hospital complex of São Paulo University Medical School

Abstract

The cases of Acute Kidney Injury (AKI) are rising both in our field and in the world. This is due to the increase in the survival of critically ill patients who are submitted to multiple nephrotoxic agents, such as contrasted exams and medications, as well as their own underlying causes, such as surgeries, shock and rhabdomyolysis. In Brazil, we also have the causes of AKI due to tropical diseases (leptospirosis, yellow fever, malaria and accidents caused by poisonous animals). In the occurrence of AKI, there is a significant increase in mortality, length of hospital stay, and costs, regardless of the reason for hospital admission. Mainly in the United States, there is a very detailed and rigorous case series, demonstrating the epidemiology, causes and mortality of AKI. The BEST study also exposed the epidemiology of several centers around the world. At the time of the study, our hospital was also contemplated. However, in Brazil, data collection is still unsatisfactory. The Clínicas Hospital of São Paulo University Medical School is the largest center for care of AKI patients in Latin America, because it is a tertiary care hospital. Nevertheless, the data of patients seen is still on paper sheets. Although the hospital has had electronic medical records for some time, due to structural problems (such as wifi), our medical records are still restricted to paper. We propose to collect the main data of the patients so that it does not remain in paper and can be computerized. This will be a massive work, since the collection will involve data from 2015 to 2017. The gathering of this data is fundamental, because then we will have the characteristics of the patients who develop AKI. Further than this collection being able to produce national and international publications, it will also be of fundamental importance for possible changes of conduct and therapy. The data will be placed in Excel table for further statistical analysis.
