Association between strength, kinesiophobia and hip and knee kinematic during single-leg landing in people with patellofemoral pain: a cross-study study

Abstract

Evidence shows that people with patellofemoral pain (PFP) have strength deficits in the hip and knee muscles, as kinesiophobia. These changes may contribute to have compensatory movement patterns with the purpose to decrease the patellofemoral stress and the pain. The decrease of hip internal rotation and knee flexion during the support phase of functional tasks, observed by some studies in people with PFP, may be a strategy to reduce the patellofemoral stress. An absence of association between the knee extensor strength and the knee flexion during descent stairs has already been observed, but the highest kinesiophobia was associated with the lowest knee flexion. However, it isn't known how these variables are related during functional tasks of greater demand. In addition, it isn't known how the hip strength and the kinesiophobia are related with the hip movement in transverse plane. Thus, the objectives of this study are: 1- to verify if there is an association between hip extensor torque, the kinesiophobia and the hip movement in the transverse plane in people with PFP during a single-leg landing; 2- to verify if there is an association between knee extensor torque, the kinesiophobia and the knee movement in the sagittal plane in people with PFP during the same functional task. Twenty-six women with PFP, aged 18 to 45 years, will participate in the study. The hip and knee extensor torque will be evaluated using an isokinetic dynamometer. The kinesiophobia will be evaluated by Tampa Scale of Kinesiophobia questionnaire. A three-dimensional analysis of a single-leg landing will be performed. The Pearson or Spearman correlation coefficient will be used to determine the association between the variables (significance level = 0,05).
