Adherence of patients with chronic pain in the shoulder to the home exercise booklets: barriers, facilitators and impact on pain, disability and self-efficacy

Abstract

The shoulder is a joint that often presents reports of musculoskeletal pain, which can be persistent and incapacitating for the individual to perform their daily activities. Treatment methods, such as primer of home exercises, are being studied because they have several benefits. However, it is still necessary to investigate the factors that determine the patient's adherence to a home program based on exercise primer without the mediation of a health professional, as well as to determine whether the level of adherence influences some clinical outcomes. Thus, the objectives of this study are to investigate the level of adherence of patients with chronic shoulder pain to a primer of home exercises performed without the mediation of a health professional, describe the barriers and facilitators for adherence, and determine whether the level of pain shoulder and disability, kinesiophobia and self-efficacy in managing chronic pain are predictors of the level of adherence to the home exercise book. All individuals with chronic shoulder pain who are waiting for the Shoulder Complex Rehabilitation Clinic (ARCO) and who will receive the exercise booklet at home will be recruited. The Shoulder Pain and Disability Index (SPADI) scale will be applied to measure shoulder pain and disability, the TAMPA Scale Kinesiophobia questionnaire (TSK) for kinesiophobia, and the Pain Self-Efficacy Questionnaire (PSEQ-10) for self-efficacy. Adherence to the home program carried out using a booklet will be assessed using the Exercise Adherence Rating Scale (EAAE-Br), which is considered acceptable for scores of at least 17/24. Descriptive analysis will be carried out for the sample characterization data, degree of adherence to the booklet, and barriers and facilitators. Multiple Linear Regression will be performed to determine the influence of shoulder pain and disability, kinesiophobia, and self-efficacy on the level of adherence to the booklet.(AU)
