The impact of the time interval elapsed between simptoms, diagnosis and beginning of treatment of women with uteru's malignant neoplasms atended in CAISM in 2010 to 2017

Abstract

Malignant neoplasms of the uterine body are prevalent, especially in developed countries. The region of Campinas (SP) is developed and shows an increasing trend in these neoplasm cases in a previous study. Long waiting periods were detected for the diagnosis and treatment onset of the Type 1 neoplasms, with a better prognosis, although without impact on staging and survival. The question that remained is about the long waiting periods related to Type 2 endometrial neoplasms (more aggressive and makeup one-third of the cases) if would have an impact on outcomes. Women's Hospital at Unicamp is the regional reference for oncologic gynecological care by SUS. The aim of this study is to evaluate the impact of the time elapsed between the onset of symptoms and diagnosis or treatment over the staging and survival of women with uterine neoplasms (Types 1 or 2). A cohort study will be conducted with cases originating in cities from the region of Campinas between 2010 and 2017. The sample size calculated is 134 cases per group considering survival rates of 75% if Type 1 and 48% if Type 2, and the errors alpha = 5% and beta = 20%. There are about 240 cases Type 1 and 150 cases Type 2 cared in the targeted period. The selection will use the Hospital Registry of Cancer and CID-O C54 (cancer of the uterus). The patients' charts will be reviewed and a database will be set up with information about the year of diagnosis, city, age, symptoms, diagnostic method, histological type and grade, staging, the time elapsed for diagnosis and treatment, type and result of treatment, recurrence and final patient status. Descriptive analysis will be performed by the histological group and the relationship between the time elapsed for diagnosis and treatment over staging and survival. Chi-square (or Fisher) and t-student tests will be used, and Kaplan Meier survival curves will be built and analyzed by the Log-rank test.
