Abstract

Older people are susceptible to the occurrence of adverse drug events (ADEs). Triggers are valid strategies for detecting ADEs, however, evidence is limited about their performance and applicability in real-time and cost analysis. Objective: To evaluate and compare the performance and applicability of triggers for the detection of ADEs in hospitalized older people and associated costs. Methodology: This project will be conducted in two steps. Step 1: Systematic review in PubMed, Scopus and Lilacs databases to identify scientific evidence about the performance and applicability for the detection of ADEs in older people. Step 2: Pharmacoepidemiological study with retrospective and prospective analysis with older people (e 60 years) admitted for more than 48 hours in a Brazilian hospital. Thirty-two triggers will be evaluated for the detection of ADEs and 21 triggers for the identification of hospitalizations resulting from ADEs by calculating the positive and negative predictive value, sensitivity and specificity. Among eligible older people, electronic medical records will be analyzed for screening of triggers, data and variables collection and pharmacotherapy review. Direct medical costs (associated with medical interventions) of older people with potential ADEs will be estimated, considering the perspectives of the National Health System (Brazil) and hospital. For cost analysis and identification of possible risk factors associated with ADEs, logistic regression models will be designed. Expected results: The triggers with good performance and applicability can be incorporated into clinical practice through protocols for the prevention, detection, management and report of ADEs. As for cost estimation, it will assist in planning resources dedicated to older people with ADEs. Finally, the divulgation of the technical-scientific knowledge of the data obtained for the population, professionals and health managers will be carried out. (AU)
