Abstract

Changes in postural responses during external perturbations represent a significant factor contributing to falls in individuals with Parkinson's disease (PD). Given the limited effectiveness of pharmacological interventions in enhancing postural control, complementary therapies such as transcranial direct current stimulation (tDCS) have been explored. While tDCS has shown positive effects on postural control in people with PD, it remains uncertain whether a single session of tDCS can enhance postural responses in individuals with a history of falls. Therefore, this study aims to analyze the effect of tDCS on the postural responses of people with PD with and without a history of falls. The study will utilize data gathered from a PhD project funded by FAPESP, involving twenty-four PD participants. These participants are categorized into two groups based on their fall history within the preceding 12 months: fallers and non-fallers. Anodal tDCS was applied to the primary motor cortex for 20 minutes before evaluating postural control under two conditions: active (2 mA) and sham (placebo). Stimulation sessions occur on different days, and the order is randomized. After tDCS, participants undergo seven trials involving external perturbations induced by posterior translation of the support base. These perturbations are executed to assess postural control. Electromyographic and center of pressure parameters are analyzed to evaluate postural responses following the perturbation. ANOVAs with factor for group (faller x non-faller) and stimulation condition (active x sham) with repeated measures for the last factor will be used for statistical analysis.
