Sleep analysis in participants of exercise-based cardiac rehabilitation programs: relationship with the occurrence of signs and symptoms

Abstract

Introduction: During the practice of physical exercise in cardiac rehabilitation programs (CRP), participants may present signs and symptoms, which may signal the occurrence of major adverse events. Sleep disorders, such as obstructive sleep apnea and insomnia, reduction in sleep duration, and sleep complaints are considered risk markers for the occurrence of major adverse events in individuals with cardiovascular disease (CVD). Factors such as these can assist in identifying individuals who are more likely to have signs and symptoms during CRP sessions. It is unclear in the literature if sleep-related aspects can estimate the occurrence of these complications during exercise. Aim: To investigate the relationship between sleep-related aspects and the occurrence of signs and symptoms in CRP. Methods: This is a longitudinal observational study. Individuals with a CVD diagnosis and/or CVD risk factors participating in exercise-based CRP will be included. In the first phase, the sample characterization assessment and the sleep assessment will be made (actigraphy: total sleep time, sleep efficiency, sleep latency, wakefulness after sleep onset and number of awakenings; Questionnaires: Pittsburgh Sleep Quality Index, Insomnia Severity Index, STOP-Bang and Epworth Sleepiness Scale). In the second phase, participants will be accompanied by 24 CRP sessions for the signals (arrhythmias and increased systolic e200mmHg in exercise) and symptoms (angina, muscle pain, fatigue, and dizziness) assessment. Generalized Linear Models will be used to assess if sleep-related aspects are able to estimate the occurrence of signs and symptoms. It is expected that this project to obtain information that will enable to establishment of better conditions for risk stratification, monitoring and prescription of exercises, and direct future clinical interventions in CRP.
