Abstract

Mental health is a crucial topic in the study of population health today. According to the World Mental Health Report, one in eight people lives with a mental health condition, representing a global prevalence of 13% of the population, with anxiety and depression disorders being the most common. As these disorders are understood through the lens of their prolonged impacts, it is crucial that public health strategies are developed efficiently and inclusively. Integrating psychological interventions into primary care is essential to effectively meet the mental health needs of the population. It is believed that offering online psychological therapies has several potential benefits that can improve the accessibility and availability of evidence-based treatment. However, current knowledge about Internet-based Cognitive Behavioral Therapy (I-CBT), implemented in primary care settings, is still limited. Complex interventions, such as the implementation of I-CBT, involve and depend on the adoption of multiple interactive components, such as the intervention itself, as well as the organizational structure and training of professionals. Therefore, the present study aims to develop, implement, and evaluate a model of education in Internet-based psychotherapy (I-CBT) for the training of psychologists in Basic Health Units in the cities of Indaiatuba and Jaguariúna. To achieve this, a hybrid type II implementation and effectiveness study, with a mixed-methods approach, will be conducted with the collection of qualitative and quantitative data. Considering the implementation of I-CBT in the cities of Indaiatuba and Jaguariúna through the CONEMO app, the present study will be developed in five stages: Mapping the network psychologists; Developing I-CBT training for Basic Health Unit psychologists; Implementing the training; Supervising the psychologists; Evaluating the effectiveness of the implementation. The training will be developed based on the mapping of the Basic Health Unit professionals' demands, considering the evaluation of previous knowledge in I-CBT, the approaches used by psychologists in the work environment, and the availability of the professionals involved. The training will be aimed at Basic Health Unit psychologists and will present evidence-based and best practice-based conduct, considering the principles of SUS and respecting aspects sensitive to primary care. It will also address the role of therapists in the use of Internet-Based Psychotherapy by Basic Health Unit users. Twenty-eight Basic Health Units (UBS) in the cities of Indaiatuba and Jaguariúna will be included. Therapists conducting Internet-based psychotherapy (I-CBT) through the CONEMO app must have a degree in Psychology but not necessarily a specialization in Cognitive Behavioral Therapy. Regarding data analysis, for qualitative data collection, focus groups with the psychologists involved will be conducted to explore their perceptions and experiences in implementing the training. Additionally, semi-structured interviews related to the implementation processes will be conducted. Psychologists' self-assessment of their knowledge in I-CBT will be carried out after the training. For the collection of quantitative data, the instruments used will be prepared in the form of electronic forms through the REDCAP program, which allows the application of validated scales and evaluation questionnaires online. It is expected that the implementation of the training will be able to increase proficiency in the use of I-CBT in the context of primary care and knowledge of specific I-CBT intervention protocols among the professionals involved, as well as help increase the number of patients treated in the Basic Health Units of these cities compared to traditional clinical care.
