Analysis of Chronic Stress on the Periapical Inflammatory Process.

Abstract

Primary endodontic infection (PEI) is complex and involves a polymicrobial profile that can lead to apical periodontitis (AP). This periapical inflammatory response is mediated by cytokines such as IL-1 and TNF-alpha, which regulate bone remodeling and osteoclastic activity. Chronic stress alters body homeostasis and the immune system, potentially negatively impacting endodontic treatment. Therefore, endodontic treatment must consider the management of systemic and inflammatory factors that can influence its success. The aim of this research is to evaluate the influence of stress, through biomarker measurement, on the periapical inflammatory process by analyzing pro-inflammatory cytokines. Uniradicular teeth with PEI and AP will be selected, and samples of periapical fluid will be collected after biomechanical preparation (SF) to quantify inflammatory mediators (IL-1B, IL-6, IL-8, IL-17, and TNF-alpha) using a multiplex assay. Additionally, stress markers will be analyzed before treatment through saliva collection at three daily time points, which will subsequently be subjected to a multiplex assay for analysis of cortisol, beta-endorphin, and ACTH hormones. Data will be evaluated using descriptive and inferential statistical tests, depending on the obtained data, with either parametric or non-parametric tests.
