Abstract Highly Active Antiretroviral Teraphy (HAART), which inhibits viral replication and transmissibility, together with universal treatment access policies, has been increasing the survival rates of people living with HIV (PLHIV) and, consequently, their aging. However, there is a high incidence of HAART-related side effects associated with metabolic changes. Chronic inflammation linked to HIV, associated with the development of insulin resistance, endothelial dysfunction, lipid metabolism alterations, liver diseases, kidney diseases, and hormonal disorders, along with HAART's side effects and physiological aging, contribute to the emergence of chronic degenerative comorbidities. Thus, an increase in non-AIDS-related comorbidities has been observed among nonelderly PLHIV, particularly metabolic syndrome (MS). The main objective of this study is to evaluate the occurrence of MS in PLHIV under 50 years old, as well as its risk factors, in follow-up at the Specialized Infectology Outpatient Service"Domingos Alves Meira" (SAE-DAM), a reference center in the interior of the state of São Paulo. This is an observational and cross-sectional study that will assess 91 PLHIV under 50 years old who are in follow-up at SAE-DAM. These patients will be evaluated regarding clinical, epidemiological, and laboratory characteristics. It is expected that, through the study results and identification of MS prevalence in the non-elderly HIV population, new prevention approaches and appropriate treatment strategies can be proposed to improve the quality of life of this population.
