At-home bleaching using universal prefilled whitening trays compared with customized trays: randomized clinical trial

Abstract

The aim of this randomized clinical trial is to evaluate the performance of at-home bleaching with universal prefilled whitening trays (PT) compared with customized trays (CT) regarding tooth bleaching, gingival condition, tooth sensitivity, enamel demineralization and volunteer satisfaction. 10% Hydrogen peroxide (HP) and 10% carbamide peroxide (CP) peroxide-based gels were tested. Forty five volunteers were recruited and randomly divided into three treatment groups: OGO (Opalescence Go – 10% HP in PT); OPF (Opalescence PF – 10%CP in CT) and WTC (White Class – 10% HP in CT). The bleaching treatment were accomplished 1x/day for 14 days, during 2 h for CP and 30 min for HP. The teeth shades were evaluated visually using the shade guides Vitapan Classical (VC) and Vita Bleachedguide 3D-MASTER (VB), and spectrophotometrically. The tooth sensitivity were evaluated using a numeric scale associated with a visual analogue scale; the gingival condition with the Löe’s index; the enamel demineralization with a fluorescent laser irradiation and volunteer satisfaction with a questionnaire. The evaluations were performed in different moments of the treatment. Data of tooth bleaching were submitted to ANOVA repeated measures and Tukey’s multiple comparison test (5%). For both tooth sensitivity (TS) and gingival inflammation (GI), the absolute risk was calculated using Fisher’s exact test and for intensity comparison among the groups, ANOVA repeated measures for TS and Kruskal-Wallis for GI were applied (5%). The degree of mineralization consisted of descriptive statistic and the answers about questionnaire were in percentage. Regarding to color change, the values obtained by VC and VB shade guides did not present significant difference for the analyzed factors (p>0,05). For ΔE data, the groups presented similar values (p>0.05) as well as the ΔL, Δa e Δb results (p>0.05). For TS, in absolute risk determination, the groups were similar among them (p>0.05). For sensitivity intensity, comparing the groups, there was no significant difference for time and interaction factors (p>0.05), but bleaching factor presented significant difference (p=0.026), in which OGO promoted higher sensitivity intensity than WTC. For absolute risk of GI, there were difference among the groups (p<0.05). OGO presented higher frequency of GI than WTC and OPF did not presented GI. For GI degree, the IG intensity was similar among the groups after bleaching (p=0.232). Regarding to mineralization, data were restricted to values that correspond to healthy teeth. It was concluded that the color change was similar and promoted patient’s satisfaction with all bleaching protocols tested. Tooth sensitivity occurred in all groups, but decreased progressively over time and the intensity was higher in HP in PT than HP in CT. Gingival inflammation only affected the PH groups and no influence of type of trays was detected. The bleaching gels did not promote alteration in enamel fluorescence. (AU)
