Obstructive sleep apnea treatment in stroke patients: a randomized clinical trial

Abstract

Obstructive sleep apnea (OSA) is present in more than 70% of acute stroke patients, and has been related to poor short-and long-term outcomes. Therefore, studies to assess the benefit of early treatment of OSA become essential in this setting. The treatment of choice for OSA is the use of CPAP (continuous positive airway pressure). Results regarding the feasibility and safety of CPAP in post-stroke patients are encouraging. However, studies of effectiveness are scarce and controversial, which justifies the proposal of this project. Evidence of the benefit of OSA treatment in stroke patients will dramatically change daily clinical practice. Therefore, our primary objective is to evaluate whether early treatment of OSA with CPAP in ischemic stroke patients has a favorable effect on functional evolution. We will perform a prospective, multicenter, randomized study, including 170 patients diagnosed with ischemic stroke and severe OSA. They will be randomized in a ratio of 1:1 (CPAP+ vs. CPAP-), and followed for 6 months. This project aims to create a new research group in the Institution with a multiprofessional team (physician, physiotherapist, speech therapist, nurses, psychologist). The proposal is the formation of an Interdisciplinary Research Center on Sleep and Stroke (IRCSS), which will contribute to the development of projects that involve sleep disorders in the context of cerebrovascular diseases. Besides having a multidisciplinary team, this Center will allow interaction with other national and international researchers, expanding the research on this field not yet widely exploited in Brazil.
