Biomechanical analysis and systematic review about the effect of occlusal splint on dental implants in cases of tooth clenching.

Abstract

Occlusal splints consist of one of the treatments for teeth clenching and can be indicated after rehabilitation with implant-supported prostheses. Thus, the aim of this study is to evaluate the effect of the materials and their thicknesses used in the manufacture of the occlusal splint on single and three-element implant-supported prostheses in a teeth clenching situation. Twenty three-dimensional models will be simulated in the programs In Vesalius, SolidWorks and Rhinoceros and will be constituted of a maxillary bone block referring to the region from the 1st premolar to the 1st right molar, with the presence of three external hexagon implants of 4mm in diameter and 7mm in length that will support metal-ceramic prostheses with three screwed elements, single or splinted, with or without using the occlusal splint, this one made of acetate, acrylic resin or polyether-ether-ketone and modeling thickness of 1, 2 and 3 mm. The ANSYS 19.2 program will be used to generate the finite element models in the pre- and post-processing steps. Bone tissue will be analyzed by Map of Maximum Principal Stress and Microdeformation and fixation screws and implants by von Mises Stress Maps. In addition, a systematic review will be performed, with probable meta-analysis, according to Cochrane Group model, to understand the effect of the occlusal splint after these similar cases of rehabilitations. The results will be interpreted in order to improve the indication of using the occlusal splint in the clinical routine. (AU)
