Effect of L.acidophilus LA5 on the progression of periodontitis. Longitudinal study in humans: clinical, immunological and microbiological parameters.

Abstract

Probiotic therapy is a promising strategy for controlling periodontitis and other inflammatory diseases. However, the lack of standardized protocols with proven clinical efficacy makes its clinical application difficult. The success of probiotic therapy is dependent on genetic factors, diet, resident microbiota, age and clinical parameters. In pre-clinical studies, funded by FAPESP, the L.acidophilus LA5 strain showed promise results for controlling periodontitis, due to its anti-inflammatory capacity, its effect on periodontal pathogens, by favoring oral and intestinal rebiosis, and controlling reabsorption. of alveolar bone in experimental models of periodontitis. Thus, we aimed to evaluate the effect of oral administration of LA5 on the control of periodontitis in 86 participants under supportive therapy at FOUSP, in a double-blind, randomized, 3-month clinical trial. LA5 probiotic strain will be administered as oral dilution sachets and capsules, aiming for benefits in the mouth and intestine, with the prebiotic inulin whereas the placebo group will receive only the prebiotic. Aiming to determine the factors associated with the success/failure of the therapy, the effects of administration of the probiotic strain will be evaluated in patients treated mechanically with and without antibiotic therapy, with different probing depths, aged more than 18 years, and with various immunological and microbiological profiles. Periodontal clinical parameters, oral microbiome, and salivary levels of cytokines, chemokines, defensins, RANKL, and pro-resolving mediators will be determined at baseline and after 3 months of probiotic or placebo administration.
