Quality of life evaluation, salivary cortisol and amylase levels and electromyographic activity of masticatory muscles: associations with parafunctional and sucking nutritive habits in children

Abstract

Assessment of stress salivary markers has gotten increasing interest in biomedical and psychological researches. The free cortisol measurement and, recently, the alpha-amylase activity, have demonstrated to be important biomarkers of the endocrine activity in physical and psicossocial stress conditions. The literature suggests that stress and emotional disturbances may be involved in the etiology of stomatognatic parafunctions, like bruxism and persistent sucking habits in children. In this context, it is important to consider the children"s oral health, because of the large number of orofacial disorders that can compromise the function of stomatognatic system, as well as their comfort and quality of life. Thus, this study project is related with three researches, as well as future subsequent researches that will be done in this area. The first study (Postdoctoral project; Fapesp n. 06/06338-0) will evaluate 150 children in the mixed dentition, aged 6-8 years, divided in two groups according to gender. The following variables will be verified: presence/absence of parafuncional habits (bruxism, nail biting, finger/thumb/lip sucking, pacifier), breast- and bottle-feeding, malocclusion; quality of life evaluation scale; body variables (weight, height and heart rate); electromyography of masticatory muscles and bite force; salivary levels of free cortisol and alpha-amylase; aiming to determined the relation among parafunctional habits and local (occlusion) and central factors (hormones and psychological factors). Second study: the purpose will be to evaluate 30 children of both genders aged 84-95 months (7 years), in the mixed dentition, without history of invasive dental treatment (restoration or tooth extraction) or pain of dental origin. The following variables will be verified: salivary levels of free cortisol and alpha-amylase 30 minutes after awakening (baseline), and before and after dental prophylaxis, as well as heart rate, body variables (weight and height) and hour"s sleep. The third study will evaluate 200 children, aged 8-12 years, from public schools and those who were to start dental treatment at Piracicaba Dental School (UNICAMP). The following variables will be verified: 1- dental variables (fluorosis, gingival health, DMF-T and dmf-t index, dentofacial anomalies; 2- functional variables: signs and symptoms of temporomandibular dysfunction; psychological factors: oral health related to quality of life, evaluated by questionnaires CPQ8-10 and CPQ11-14; 4- salivary levels of cortisol and alpha-amylase. Longitudinally, 40 children who have presented similar dental conditions, but different impressions of their oral health will be re-examined and interviewed intra-individually. In this way, the results will be submitted to descriptive statistical analysis, Shapiro-Wilks-W test, Pearson"s and Spearman"s correlation tests, t-test, linear multiple regression and stepwise logistic regression, where appropriate.
