Since 1996, Highly Antiretroviral Active Therapy (HAART) reduced the mortality and morbidity of HIV-1. Furthermore, we had the emergence of resistant viral populations, as a consequence of lack of adherence to treatment, pharmacokinetic dependent on host genetics, tissue sanctuaries, among others. The search for new drugs directed at different targets of the viral cycle is essential for use in salvage therapy for individuals in the HAART failure. The integration of the genome of HIV-1 to the host is a key step in the establishment of a viral reservoir that represents a drawback to the eradication of this virus. Raltegravir is the first integrase inhibitor to be approved by the FDA for use in antiretroviral treatment-experienced adults patients with viral resistance. Raltegravir blocks HIV replication by inhibiting essential strand-transfer activities of integrase. Along with this, we have the presence of primary mutations to certain antiviral drugs even without the individual having received them as a result of its transmission. Brazil has a program of free distribution of antiviral and the São Paulo city was the precursor. Thus, the aim of this study is to standardize a test for assessing the presence of resistance to antiretroviral Raltegravir and evaluate the presence of primary resistance mutations in individuals in the HAART failure that are eligible to receive Raltegravir.
