Characterization of the profile of microvolt T wave in hypertrophic cardiomyopathy

Abstract

Hypertrophic cardiomyopathy (HCM) is the primary disease of the heart muscle whose characteristic is the presence of ventricular hypertrophy that occurs in the absence of apparent cause, cardiac or systemic. Of genetic origin, transmitted in an autosomal dominant with variable penetrance, has an estimated prevalence of 1:500 in the general population is of great importance because it is the main heart disease responsible for sudden cardiac death (SCD) in young adults and athletes, and generally involves asymptomatic individuals under the age of 55 years. Ventricular tachycardia and ventricular fibrillation (VT / VF) are responsible for SCD in patients with HCM and the possibility of identifying patients at higher risk of occurrence of these ventricular arrhythmias is a major challenge today and also is essential for the development strategies for prevention of SCD in this population. Some clinical factors have been known as having the worst prognosis and identify individuals at high risk for the occurrence of SCD or history of cardiac resuscitation for ventricular fibrillation and ventricular tachycardia, family history of premature SCD, unexplained syncope, ventricular septal thickness e30 mm nonsustained ventricular tachycardia and inadequate response of blood pressure to exercise. In the quest to find a reliable diagnostic method that can assess this risk appeared to micro-T wave alternans (MAOT), a noninvasive, inexpensive and easy to perform, which is already used to evaluate the occurrence of these arrhythmias and stratification of SCD and other heart diseases, such as in ischemic heart disease and heart failure, and that has value unknown in HCM. This research aims to evaluate first the presence of MAOT in HCM and its association with clinical factors for SCD, with the possibility of emerging as a new method that would help to predict more accurately the risk of SCD in this population.
