Abstract
Caries diagnosis has become a difficult task in dentistry. New tools have been developed aimed at helping the conventional methods (visual and radiographic) in the detection of incipient caries lesions on occlusal and approximal surfaces. Some of these methods are based on the light-induced fluorescence phenomenon, such as DIAGNOdent, DIAGNOdent pen, QLF and the intra-oral camera VistaProof. Several in vitro and some in vivo studies have been carried out on the performance of these methods using occlusal and approximal surfaces. However, there is a lack of information about the performance of these methods in detecting incipient non cavitated caries lesions on smooth surfaces. Therefore, the aims of the present study are: (1) to develop an in vitro protocol to induce the formation of incipient non cavitated caries lesions on bovine enamel blocks using a cariogenic bacterial model and (2) to evaluate the performance of fluorescence-based methods in detecting and monitoring incipient non cavitated caries lesions formed in vitro. Ninety bovine teeth will be used. They will be cut and polished. Superficial microhardness will be measured and the blocks will be submitted to a protocol of incipient non cavitated caries formation using S. mutans and S. mutans associated to L. casei and cariogenic solution. Superficial and longitudinal microhardness will be measured again, as well as polarized light microscopy will be performed, which will be used as gold-standard for the presence of non cavitated lesions. Thus, these results will define the interval needed to induce incipient lesions and make possible the evaluation of the fluorescence-based methods in their detection. Moreover, the interval for the lesion's evaluation (monitoring) will be also defined. In the second part of the study, 40 bovine teeth will be used and prepared as described above. Superficial microhardness and baseline analysis of the enamel blocks using the fluorescence-based methods (DIAGNOdent, DIAGNOdent pen, QLF and intra-oral camera VistaProof) will be performed by 2 examiners. Afterwards, the blocks will be submitted to the induction of non cavitated caries lesions as described above. A second analysis will be performed using the methods cited in order to evaluate the performance of these methods in detecting and monitoring the lesions. Superficial and longitudinal microhardness measurement as well as polarized light microscopy will be performed. Statistical analysis will be carried out by means of ROC curve analysis, values of sensitivity, specificity and accuracy (compared by the McNemar test). Reproducibility will be assessed by means of Intraclass correlation (ICC). Depending on the data distribution, either Student T- or Friedman test will be used to compare the values before and after demineralization. (AU)
