Seric levels of manose-binding lectin (MBL), metabolic control and nefrophaty in Diabetes mellitus

Abstract

Diabetes mellitus is a systemic and chronic disease. In diabetics,serious and more frequent infectious diseases with the involvement of opportunist agents are frequent. Some papers have shown some rank of secondary immunological compromise. MBL is a protein from the family of the colectinas. MBL active the classical road of the complement after the connection of the globular portion to residues of oligo and polissacárides. The activation of complement mediated by the MBL represents a third road of distinct acceleration of the alternative and classical roads. The deficiency of MBL has been considered the most frequent cause of immunodeficiency. Publications suggested that MBL can modulate the gravity of the infectious and autoimune illness. With respect to diabetes mellitus, conflicting results have being shown about seric levels and also the analysis of polymorphism. Several studies has suggested the possibility of levels elevated of MBL will be associated to the development of nefropathy and arteriosclerosis in diabetics. Our group studying children and adolescents with diabetes did not find increase of infectious incidence. However, some immunological parameters were modified in these diabetics, mainly associated to the increased level of glucated hemoglobin (HbA1c). As the levels of MBL are dependent of the connection with sugars and potentially can be limited by the action of insulin, would these levels suffer influence of glucose or at least by the variation of the HbA1c? We are going to measure the seric levels of MBL and correlate with metabolic control measured by glycated hemoglobin, number and kind of infections and 24hs microalbuminuria in type 1 diabetics with ages between 5 and 15 years.
