Abstract

Introduction: Great emphasis has been placed on the prescription of aerobic physical exercises in cardiovascular rehabilitation (CR), however, although these are the most widely recommended, when one speaks of promoting health, there is a growing emphasis on resistance exercises in coronary arterial disease patients (CAD), which are essential for the performance of both occupational and recreational daily activities and leisure. However, the knowledgement of cardiocirculatory and peripheral metabolic adjusts in this modality has been poorly studied. Principal Objective: The objective of this study is to investigate the autonomic, ventilatory and metabolic adjustments in dynamic and resistance exercise in Leg Press 45° and compare the responses between healthy with coronary arterial disease subjects. Secondary objective is to determine a metabolic transitional point, as determined by lactataemia, heart rate and ventilatory variables, contrasting different intensities of resistance exercise. Additionally, a thirty objective is to verify if resistance physical exercise associated to aerobic training, based on anaerobic threshold can improve strength and muscle endurance in this patients. Study Design: Prospective study, longitudinal, controlled trial. Subjects: 20 male patients diagnosed with CAD, clinically stable, with at least 12 months since the acute event, after surgical or percutaneous revascularization and 6 months of CR with a functional capacity of >7.5 Mets and 10 healthy active volunteers matching in age (control group). Methods: The participants will undergo an incremental clinical test on ergometric cycle and a test of 1RM on a 45° leg-press, and subsequently, on separate days, a physical exercise on ergometric cycle with discontinuous dynamic exercise tests (DDET) with degrees of 10% of workload obtained during incremental test. Additionally, will be applied a resistance physical exercise tests (RPT) at different percents of 1MR, from which the HR, blood lactate and expired gas responses will be evaluated. Study Hypothesis: Autonomic responses during cicloergometer and resistance exercise would be associated to muscle metabolism during transitional fase of energy supply (anaerobic threshold) in subgroups studied, 2) metabolic responses during both physical exercise (ciclo and resistance) would be associated to ventilatory responses in both groups; 3) in DAC subjects, the ventilatory, cardiovascular and metabolic adjustments would occur early when comparing with healthy matched controls, 4) The methods to anaerobic threshold would be concordance between methods to determination of anaerobic threshold in dynamic and resistance exercise in both groups; 5) Resistance and aerobic training based on aerobic-anaerobic transition would improve endurance and strength and aerobic capacity in these patients. Study Relevance: The establishment of percentage of maximum resistance based on responses of HR and their HRV, lactataemia and expired gases, as well as how to detect AT through different methodologies will make it possible the knowledgement of physiological responses during different percentages of maximal resistance in healthy subjects and CAD patients and obtain practice foundation to prescription of resistance exercises and elaborate physical therapy treatment to reconditioning the peripheral musculature of these patients.
