Correlation between IGF-1 and IGBP-3 serum levels and the muscle volume and performance in chronic hemiparetic individuals

Abstract

According to National Health Department, stroke is the main cause of death in Brazil amounting 70.232 deaths in 2008. Likewise, in conformity with the World Health Organization (WHO), it is the primary cause of disability in the world. The Brazilian population ageing and great predominance of risk factors have contributed to a gradual increase in the number of cases in the country, turning it into an extremely important problem of public health. The post-stroke rehabilitation is generally a long, complex process. Muscle weakness (paresis) is one of the most significant alterations concerning disability and its causes have not yet been entirety described. The understanding of such muscle alterations is relevant to the development of therapeutical strategies. In this context, the objective of this study is to evaluate the volume and the performance of the extensor muscles of knee in chronic hemiparetic individuals and correlate these two factors to the IGF-1 and IGFBP-3 serum levels, which are biomarkers of muscle trophism. For that purpose, a transversal study will be developed by analysing 26 individuals (aged 40-70, of both sexes) who will be divided into two groups: 1) The Control group with healthy individuals; and 2) The Hemiparetic group. The performance of knee extensor muscles will be evaluated in concentric and eccentric contractions at 60o/s in isokinetic dynamometer. The variable investigated will be the torque peak and the average power. The volume of the quadriceps muscles will be obtained by means of Nuclear Magnetic Resonance (NMR) and the activation of extensor muscles of the knee will be attained by electromyography (EMG). The IGF-1 and IGFBP-3 serum levels will be analysed by the ELISA method. All individuals will be asked to go through the Fugl-Meyer Assessment of Sensorimotor Recovery. For statistic purposes, the tests of normality (Shapiro Wilk's test) and homogeneity (Levene's test) will be applied. The Anova-one-way test followed by Tukey will be utilised in the case of parametric data. Otherwise, the non-parametric statistic Kruskal-Wallis. Either Pearson's or Sperman's test will employed for correlation analyses. The 0.05 significance level will be considered as well as the 95% confidence interval (CI) for all statistical tests.
