Assessment of primary heath care in the treatment of TB from the perspective of health professionals and patients in Brazilian cities

Abstract

The objective of the present research project was to assess Primary Health Care Services for treatment of tuberculosis (TB) in cities of Brazil. The method used included both the quantitative and the qualitative approaches. The cities were chosen based on the model of TB care in different regions: Ribeirão Preto (SP), São José do Rio Preto (SP), Uberaba (MG), Rio de Janeiro (RJ), João Pessoa (PB) Cabedelo (PB) Natal (RN), and Foz do Iguaçú (PR). The study population consisted of TB patients and health professionals involved with the treatment of TB, and were key informers. In the quantitative approach data were collected through primary sources (structured interviews) and secondary sources (TB-WEB and SINAN information systems, medical records, and registration forms). For the interviews with the professionals a structured tool was used to collect data on the service regarding structure, process, and results, which were assessment categories and were weighed differently and classified as "satisfactory" or "unsatisfactory". The content of the tool was validated so as to verify items and ratings adequacy. A Multiple Correspondence Factorial Analysis was used to find association between components of the structure and process with treatment outcome. Patients undergoing treatment completed a structured questionnaire to evaluate attention required and received and the data collected were analyzed by means of descriptive statistical techniques. In the qualitative approach, semi-structured interviews including issue-related questions were conducted with health care professionals and content analysis techniques were used.
