Prevalence of hidden caries in young adults: correlation between index ICDAS II, digital radiography and fluorescence

Abstract

The hidden caries is a challenge to modern dentistry because it is difficult to diagnosis and its prevalence in the Brazilian population is unknown. Failures in the detection of hidden caries result in tooth fractures, root canal therapy, extractions and suffering that could easily be avoided if the lesion was diagnosed early. 384 volunteers of both genders, aged 18 to 28 years will participate in this cross-sectional observational study. Using socio-demografio questionnaire, classification of carious lesions by ICDAS II criteria and risk assessment of caries by Cariogram software, intended to establish the prevalence of hidden caries and its correlation with the high risk of caries, ICDAS II criteria and severity of hidden caries. In addition, the accuracy of the diagnosis by fluorescence diode LED and digital image magnification will be established on the basis of bitewing radiographs (gold stardard for hidden decay). The analysis will be performed by four independent observers blinded to the diagnosis of the presence and severity of hidden caries. The examiner 1 will conduct a professional prophylaxis, visual clinical examination for detection of carious lesions by ICDAS II criteria and measurement of risk of caries with Carigram. The examiner 2 performs the digital bitwing radiographs (Spectra - Owand), and will determine the prevalence and severity of hidden caries lesions (control group). The diagnosis of carious lesions by fluorescence diode LED. (Spectra - Air Techniques) will be held by the examiner 3. The examiner 4 will carry out the diagnosis of hidden caries through magnification of digital images obtained with micro-camera (Einstem - RF System). With this project we intend to answer the following questions: 1) The prevalence of hidden caries in young adult patients (> 18 and <28 years) is high and progressive? 2) Patients classified as high risk for dental caries have higher prevalence and severity of hidden decay? 3) The Code 4 ICDAS II (shadowing the underlying dentin with enamel intact or slightly damaged) correctly identifies the hidden carious lesions? 4) The codes 1, 2 and 3 ICDAS II did not correlate with caries hidden? 5) The methods of diagnosis by fluorescence diode LED and digital imaging can replace the bitewing radiographs in detecting and assessing the severity of hidden decay?
