Preoperative cardiopulmonary function and the correlation with postoperative complications in patients underwent thoracotomy

Abstract

The cardiopulmonary complications after thoracotomy increase the rates of morbidity and mortality in patients undergoing this procedure. Thus, we see the importance of tests that can predict the surgical risks. These tests should identify individuals who are susceptible to postoperative complications determining which individuals do not suffer major complications arising from medical history or conditions acquired after extensive resections. The tests for predicting surgical risk include specific tests of strength, volume and lung capacity and exercise tests, and calculations performed by means of personal data and other patient-specific tests, but the more accepted as a predictor is the oxygen consumption from ergospirometry. The cardiopulmonary exercise testing field (walking and stair-climbing) is inexpensive, can be performed in any hospital, but are little used preoperatively, through them, despite being able to calculate the oxygen consumption. The objective is to evaluate if the exercise tests, pulmonary function and respiratory muscle strength are able to predict postoperative complications in patients undergoing thoracotomy, assess the maximal oxygen uptake test 6-minute walk, obtained through portable ergospirometer and validate oxygen consumption obtained indirectly by testing the ladder measured by portable ergospirometer. The study will be conducted at the Hospital of the Faculty of Medicine of Botucatu - UNESP. Will be included all patients who underwent thoracotomy signing the consent form. Volunteers will be evaluated with the aid of a questionnaire, which contains items related to personal data and risk factors for postoperative cardiopulmonary complications. Will perform the following tests: spirometry, manometer, respirometry, diaphragmatic index, 6 minute walking test and stair climbing test. Will also be applied to the pain scale, and performing measurements to predict surgical risk. (AU)
