Abstract

During pregnancy, thyroid physiology suffers various modifications in order to adapt to the metabolic and hormonal changes experienced by the maternal organism during this period. Initially, the high levels of chorionic gonadotropin (hCG) act directly on the thyroid, due to its homology with TSH, occupying its receptors. Consequently, it causes increased levels of free T4 (FT4) and leading to suppression of TSH during the first trimester of pregnancy. During pregnancy, the increasing levels of estrogen increase the values of thyroxine binding globulin (TBG), leading to increase of total T3 and T4, but without changing the levels of FT4. However, the measurement of FT4 in most laboratories, is made by indirect methods, such as immunofluorimetric that in situations of excess TBG falsely decreased values for FT4. At this stage of gestation, TSH levels rise slightly, remaining so until the end. Because of these changes, the standard reference values for women of reproductive age do not apply to pregnant women and can lead to a classification error up to 18% of cases. Similarly, the values of thyroid hormones in pregnant women also vary with the population studied, due to the sufficiency or insufficiency of iodine and selenium. The objective of this study is to evaluate the thyroid hormone, correlate them with iodine sufficiency, by urinary iodine, and serum levels of selenium, and correlate with the ultrasonographic findings of thyroid gland in pregnant women.
