Abstract

Oral anticoagulants are efficient drugs on prevention of thromboembolic events in several clinical conditions. The therapy with oral anticoagulants is evaluated by the international normalized ratio (INR) of the protrombine time. Increased INR values are associated with the risk for potentially lethal bleeding and reduced INR are related to thromboembolism risk. In this regard, proceedings that allows better monitoring of the ancioagulant therapy benefits the patient, reducing the risk for these complications. The use of warfarin, vitamin K antagonist, represents a challenge to the clinical practice, due to its narrow therapeutics index and to the difficulty of dose adjustment due to its pharmacokinetics and genetics characteristics, interactions with other medications or foods, and other environmental factors. The aim of this study is to develop and validate the guideline of monitoring the hospitalized and outpatients in use of oral anticoagulants in a hospital unity of medium complexity. The anticoagulant monitoring protocol will be developed and validated using the Delphi method with the unidentified participation of 18 medical doctors. The development and validation of the monitoring guideline of patients in use of the oral anticoagulation will contribute to the standardization of the information exchange between healthy care professionals; the multiprofessional assistance to the hospitalized and outpatients in use of anticoagulants; the improvement of the monitoring program and elaboration of educative and follow up tools; the opportunity for under and postgraduate students develop their skills in pharmaceutical care in pharmacies, drug stores, basic health unities and other health care fields; and the development of research, knowledge and materials for health care.
