Primary intraocular lens implant for congenital cataract treatment

Abstract

Congenital cataract is the leading cause of recoverable blindness in infancy. Its early detection is done by the red reflex test, required by law in of São Paulo State since 2007. The test allows early diagnosis of congenital cataract; however we also need to adequately perform the surgical procedures and prevent amblyopia. Surgical treatment should be performed in specialized center of cataract treatment for children. Usually in the first 2 years of life, after surgery, a child became aphakic. Treating aphakic children amblyopia using glasses has a number of disadvantages that can result in low treatment adherence with inadequate visual development. Another alternative to avoid amblyopia are aphakic contact lenses, but those available in the Brazilian marked, are not viable to treat our inpatients. Therefore, the search for more effective surgical treatments to fight amblyopia more easily is justified. The intraocular lens (IOL) primary implant presents this feature; however, the high rates of complication make it a controversial procedure. Because of the current development of techniques and materials, the primary implant is becoming more and more acceptable, with encouraging results, including the decrease of glaucoma incidence rates. This project overall objective is to make our center (CTCI/FMB/UNESP) better structured for treatment of pediatric cataract. Its specific goal is to assess the safety, efficacy and effectiveness of the treatment for congenital cataract with primary IOL implantation in SUS patients, using appropriate techniques and materials for children.
