Effects of an exercise protocol in hospital fase in pulmonary function, functionalcapacity, and peripheral muscle strength of patients with left ventricular dysfunction after coronary artery bypass graft

Abstract

To evaluate the effects among different exercise protocols in hospital fase for a short period of time in pulmonary function, submaximal functional capacity, peripheral muscle strength and clinical outcomes of patients with left ventricular dysfunction (LVD) after coronary artery bypass graft (CABG). Study Design: Prospective, controlled trial. Method: 45 patients with LVD (ejection fraction <45%) underwent elective CABG will be randomized into three groups on the fírst day postoperatively (PO): Group 1 (n = 15) - Protocol I - walk, Group 2 (n = 15) - Protocol II - dynamic exercise in cycle ergometer, Group 3 (n = 15) Protocol III - peripheral muscle electrostimulation (quadriceps and triceps surae). The protocols will be applied by trained physiotherapists in the first postoperative day until hospital discharge. Spirometric data will be obtained at bedside preoperatively, and ls(, 3rd and 6th postoperative days. Peripheral muscle strength will be obtained in the preoperative and 6th postoperative days. The functional capacity will be evaluated by submaximal six minute walk test (6MWT) pre and on 6,h postoperative day. During the 6MWT a portable device connected to the patient will analyze gases - oxygen consumption (V02) peak. Every minute during the 6MWT the patients will be instructed to perform a vital capacity maneuver to analyze the flow-volume curve during exercise. Will be evaluated days of hospital stay in the PO and monitored respiratory events (pleural effusion and atelectasis) by chest radiographs performed routinely preoperatively until hospital discharge.
