The primary health care as strategy to configure the SUS: the perspective of its professionals and users

Abstract

Comprehensive knowledge is available about the difference between expectations stemming from primary health care and their effective accomplishment. This paper is particularly interested in investigating this difference as it has been hardly ever put into question. The main objective of the present study is to understand why PHC, despite all set of investments, has not managed to attain material as well as symbolic conditions to work under the centrality that is devoted to it in the health care system. Therefore, a multiple-case study under a qualitative methodology .The starting point is its main actors: managers, health care professionals, and users. Assuming that actors also produce theory, mainly explanations about the world they live and act in, so they are not passive objects before the researchers, who are supposed to be the holders of a "theory" about social reality, we made the option to work with the cartographic method, supplemented by the biographic method through life stories of users belonging the PHC unities under study. Such methodological option was chosen in a search for characterizing the senses provided by the PHC actors and their experiences, which have been labeled as "primary health care" by managers. The research will be developed in three cities in the state of São Paulo. These localities have in common the investments in expansion and qualification of a primary network. The three cities present a Consolidated public health care network that includes primary health, specialized care, emergencies, and hospital care. Three PHC unities will be studied in each city as to allow us to observe the different PHC organization models available, in addition to being appointed by the local managers. (AU)
