SPECT/CT with 99 Tc-DMSA in patients with sickle cell anemia: images and quantification of renal tubular function and compared with glomerular function

Abstract

Sickle cell anemia is a genetic disease in which hemoglobin possesses a specific mutation that alters its molecular behavior leading to vaso-occlusion in tissue and organ leading to hypoxia, renal infarction, and Progressive renal failure. Surprisingly there is a very limited data in the literature reporting the use of scintigraphy with 99mTc-DMSA to assess renal function and its correlation with glomerular filtration rate in these patientes. This study aims to compare the plasma clearance and renal uptake of 99mTc-DMSA with glomerular filtration rate measured with 51Cr-EDTA in patients with sickle cell anemia. We will study included 30 patients with sickle cell anemia, excluding vulnerable patients and pregnant patients. Ali patients will simultaneously receive doses of 10 MBq of 51 Cr-EDTA and 110 to 185 MBq of 99mTc-DMSA. Blood samples will be taken after 2, 3, 4 hours and immediately counted in a well counter in the energy window technetium-99m and after 1 week counted in the energy window chromium-51. Three hours after injection of the radiopharmaceutical we will obtain planar and tomographic images (SPECT-CT) of 99mTc-DMSA. The glomerular filtration rate calculated with 51 Cr-EDTA will be compared with 99mTc- DMSA absolute uptake and 99mTc-DMSA clearance through correlation curves. The SPECT-CT with 99mTc-DMSA will be used in the evaluation of these patients, both for absolute function assessment well as for as identifying focal lesions caused by renal infarction. (AU)
