Cost-effectiveness of Outpatient Parenteral Antimicrobial Therapy (OPAT) program based on day-hospital patients of Public Health Brazilian System

Abstract

Objective: to evaluate the cost-effectiveness of Outpatient Parenteral Antimicrobial Therapy (OPAT) program based on day-hospital for patients of Public Health Brazilian System (SUS). Methods: this project will be developed in five steps. 1. Technical Scientific Review for the TEAPA; 2. Definition of TEAPA guideline integrated to SUS regional dynamic; 3. Development of TEAPA pilot; 4. Economic analysis. Strategic terms systematic review will be "anti-infective agents", "parenteral infusion", "outpatient infusion therapy", "home infusion therapy" and "day care"; this review will include: Cochrane Library, PubMed/MEDLINE, Embase, Trip Database Center for Reviews and Dissemination, INAHTA's sites, Medscape Infection Diseases, Bireme Evidences Lilacs, BDTD bank of thesis and Google Schoolar. In the second step the TEAPA guidelines will be proposal considering the specific SUS scenario with team definition, service structure, selection of patient, therapeutic plan, follow-up plan, management of outcome and clinical governance. According defined local guidelines pilot will include 30 patients in TEAPA based on day-hospital comparing to inpatients in similar conditions. Dates obtained will be submitted to economic analysis. Economic analysis will follow recommendations of "Diretrizes Metodólogicas da REBRATS", particularly "Estudos de Avaliação Econômica de Tecnologias em Saúde (BRASIL, Ministério da Saúde, 2009). The software Tree Age Pro® 2013 will be employed for decision the construction in order to estimate costs and outcomes comparing patients in TEAPA with inpatients received antimicrobials. Incremental cost-effectiveness ratio will be calculated. (AU)
