Analysis of critical chronic patients and the impact on the quality of life among patient and their family members

Abstract

chronic critical patient is the patient who survived the acute phase of the disease and evolved chronically. They have a prolonged ICU stay associated with invasive mechanical ventilation and often tracheostomy. Generally chronic critical patients have older age, higher severity scores, major complications with high mortality rate. The return home is often suffered because of the lifestyle change, that is, require special care due to their critical health condition, often is dependent for basic care, needing caregivers. The family goes through situations of stress, change in quality of life with financial loss and problems at work. Because it is a relatively new scenario, there are few studies, mainly in Brazil. The aim of this multicenter study are: to verify the impact of the disease of chronic critical patient quality of life and emotional condition of patients and their families during and after hospitalization in the Intensive Care Unit (30 and 90 and 180 days after ICU); check the level of dependency according to the Katz index on 30 and 90 and 180 days after ICU discharge; characterize the chronically critically ill patients, for well established definitions in the literature, according to the treatment, prognosis scores and outcomes in the ICU; compare groups of chronically critically ill patients admitted to the ICU of the Hospital Sírio-Libanês,Hospital das Clinicas and Grajaú, which serves a different population as well as its pathologies. The target population of this study is chronic critically ill patients and their families, enrolled in four Intensive Care Units: general ICU of the Sírio-libanês; Medical and emergence ICU of the Hospital das Clinicas and general ICU of Grajaú Hospital. To compose the study, will be interviewed a total of one thousand families. Instruments: the HADS is used to detect symptoms of anxiety and depression; EQ-5D for the quality of life of patients and WHOQOL-brief to find the QOL of the family. The questionnaires will be applied to the family in the ICU and the follow-up will be done by phone on 30, 90 and 180 days. The family will also answer the dependency ratio of activity of daily living - KATZ of the patient. Because it is chronic critically ill patients, often with limited communication, the family will answer for themselves and also for the patient. Each center will have a person responsible for data collection and a single researcher (RRLF, HSL) will make all calls to turn this collection the most uniform as possible. Statistical analysis will be characterized descriptively and the samples obtained will be compared with patient groups of four ICUs. It will be held the Student t test for independent samples or Mann Whitney, if the variable does not present a normal distribution. To compare the means at different times, it will be held the repeated measures ANOVA and the Friedman test. The value of p <0.05 is considered significant. (AU)
