Evaluation of Recruitment Maneuver and Alveolar Collapse During PEEP Titration Assessed By Electrical Impedance Tomography and CT Scan in Dogs Undergoing Total Intravenous Anesthesia.

Abstract

The recommended protective lung ventilation present, in which use low tidal volumes (4 to 6 ml.kg-1), associated with alveolar recruitment maneuver and administration of positive end-expiratory pressure (PEEP) in order to keep the alveoli open, they must be continuously monitored. These maneuvers when misused can contribute to the formation of atelectasis, barotrauma, volutrauma and even atelectrauma; It is utterly important to your monitoring.Computed tomography (CT) is the gold standard for evaluation of pulmonary function, however, is not to be held during the anesthetic-surgical procedure or the bedside of patients in intensiv care unit (ICU). Therefore, a new technique, the electrical impedance tomography has been studied. Its operation is by emission electric current of low frequency and intensity in the tissues, administered by a belt 18 or 32 electrodes placed around the chest, between the 1st and 5th intercostal space. This does not emit radiation and there is no risk of contamination of the patient and the surgical team. The equipment has a lower cost compared to the equipment used for carrying out conventional tomography, as well as being easier to handle by being smaller may thus be used in transcirúrgico time and at bedside. This technique has recently been used successfully in the areas of pulmonology and anesthesiology human and experimental area in swine. In canine specie there is few studies to better assess ventilation during anesthesia. (AU)
