Comparative analysis of clinical and molecular parameters in implant-supported metal ceramic and hibrid ceramic crowns

Abstract

The development of CAD/CAM hibrid ceramic blocks has gained interest in implant prosthodontics due to some advantages, including ease of repair and fabrication and lower modulus of elasticity. Recent research has indicated a significant reduction of stress in the peri-implant bone with resin relative to ceramic prostheses. The aim of this randomized clinical trial will be to compare the clinical parameters and expression of bone and inflammatory markers in periimplantar fluid of the implants rehabilitated with single crowns in metal-ceramic - control or with Vita Enamic hybrid ceramic (test), at specific time points. The following evaluations will be conducted 1) symptoms of pain and oral implant mobility; 2) plaque index and periodontal probing; 3) prostheses (USPHS criteria and resin replicas for analysis in scanning electron microscopy - SEM); 4) radiography (vertical bone loss); 5) quantification of bone markers (OPG, OCN, PTH, RANKL, OPN, DKK1) and inflammatory (IL-1², IL-10, TNF-±, TGF-²). The results will be compared between groups using the following tests: Kruskal-Wallis (peri-implant parameters), Wilcoxon signed-ranks (bone loss), life table (survival of the implant), Kaplan-Meier method (estimated time between the installation of crowns and the incidence of prosthetic complications), Friedman and Mann-Whitney (bone markers and inflammatory markers), all tests with significance level of p <0.05.
