Abstract
Objective: The primary goal of this study is to compare in growing patients the dentoskeletal effects of four types of orthopedic maxillary expanders: Mini-implanted anchored rapid palatal expander (MARPE), expander with differential opening (EDO), fan-type expander (F) and conventional Hyrax expander (H). The secondary goal is evaluating the efficacy and efficiency of maxillary permanent incisor alignment comparing the invisible aligners and the fixed 4x2 mechanics. Methods: A sample of 64 patients from 7 to 11 years of age with h transverse maxillary dental arch constriction will be randomly allocated into four study groups: group MARPE composed by 16 individuals treated with hybrid expanders; group EDO with 16 individuals treated with the expander with differential opening (EDO); group F composed by 16 patients treated with the fan-type expander and; group H composed by 16 patients treated by conventional Hyrax expanders. After expander removal, patients with maxillary incisor crowding will be randomized again for treatment with invisible aligners or partial fixed appliances using a 4x2 mechanics. Cone-beam computed tomography (CBCT) will be performed before treatment (T1) and 6 months after expansion when the appliance is removed (T2). Digital dental models will be obtained at T1, T2 and T3 (after maxillary incisor alignment). Maxillary transverse dimensions, nasal cavity width, posterior tooth inclinations and buccal bone level will be measured in CBCT images. The tridimensional CBCT image of T2 will be registered in T1 using the cranial base for measuring the orthopedic effect into the three planes (x, y and z coordinates). Maxillary arch widths, perimeter, length, shape and incisor irregularity will be measured in digital models. Statistical analysis: Interphase and intergroup comparisons of maxillary expanders will be evaluated using paired t tests and Variance with Tukey tests, respectively. The efficiency and efficacy of incisor aligner mechanics will be compared using independent t tests and Chi-square tests. The significance level regarded will be 5%. (AU)
