Abstract

The ulcer of lower-extremity is a disease with high incidence and prevalence, affecting 1 to 3% of the population in developing countries. A vascular etiology is the main cause with 75 to 90% of cases. The venous ulcer of the lower limbs is the most advanced stage of chronic venous disease (CVD), being a common problem in western countries, causing significant socioeconomic impact. In Brazil, socioeconomic importance of the IVC is now considered more recently led to growing interest by the scientific / clinical and therapeutic options related to the process. Various types of dressings have been developed, with emphasis on curative bio-active. These, rich in growth factors are natural biological mediators characterized as peptides for tissue receptors. Act as regulators and disrupting the cellular activity, enzymes, angiogenic factors and anti-angiogenic inducers of gene expression, among other activities. Soluble signaling molecules are present in platelet granules that are released to promote migration, proliferation and cell differentiation contributes to the modulation of the healing response. Among the signaling molecules degranulation of platelets after many factors are identified: arachidonic acid, prostaglandin PGE2, thromboxane (TXA), platelet aggregation factor (PAF), growth factor derived from platelets (PDGF) and vascular endothelial (VEGF) than those derived fibroblast (FGF), epidermal factor (EGF) and transforming growth factor ² (TGF-²). The objective of this study is to compare the performance of hydrocolloids on the market, with a platelet gel laminar home made or emulgel, the results expected to surpass those of hydrocolloids statistically, because these biomaterials are of low viscosity that acquires firmness / consistency to be sprayed on the bed of the wound. It is technology developed in the 80s from the products to ostomies. The plates are generally composed of water, propylene glycol and carboxymethylcellulose or water and polyvinylpyrrolidone. When in contact with the wound exudate is the formation of a gel, which promotes the development of a humid microenvironment allowing the leukocyte chemotaxis and angiogenesis for optimizing the process of healing the wound. The technology using matrix platelets has been identified as promising in liuteratura specialist. This is a proposal for a randomized, blind, with the indicator: reduction in size of lesion, time of closing and symptoms such as pain and adverse effects on both products. Will be included in the study n = 35 patients from the outpatient clinic of Dermatology, Faculty of Medicine of Botucatu - Unesp, individuals of both sexes of pairs of leg ulcers of venous etiology. Monitoring of patients will be 90 days. Having observed the improvement of the lesion, but not complete healing, the patient has the right to continue being served with the product until complete healing. . For the statistical analysis of the wound area will be used for Technical analysis of variance model for repeated measures on independent groups. For the remaining data will be used to Test for Goodman contrasts between and within populations multinomiais.
