Identification of prognostic and diagnostics molecular biomarkers of cicatrization in the inflammatory exudate of chronic venous ulcers before and after treatment with fibrin sealant

Abstract

Because of the identification of individual molecular signatures in biological systems by proteomic approach, there was an increase in the discovery of biomarkers that could be used in clinical therapy as well as in the development of prognostic/diagnostic tests. The Dermatology Ambulatory from Botucatu Medical School at UNESP treats patients with chronic venous ulcers weekly and, at the moment, it has no comprehension about ulcers healing and about determination of prognostic signatures. Currently, the recommended treatments for chronic venous ulcers are not differentiated, given that, the initial efforts to implement an effective therapy is compromised when there is no information about prognosis or stage of healing in these ulcers. Therefore, the patient reports and clinical aspects are the main tools capable of assessing the degree of difficulty and healing. Nowadays, a great cientific and tecnological challenge have been assumed by researchs from The Center for the Study of Venoms and Venomous Animals (CEVAP) at UNESP in partnership with the Postgraduate Program in Tropical Diseases of Botucatu Medical School (FMB/UNESP) through a clinical trial phase I/II to evaluate the safety and best dose of the new fibrin sealant derived from snake venom in the treatment of chronic venous ulcers, which is supported and funded by the Ministry of Science and Technology (Decit) and by CNPq proc. number. 563582/2010-3. Since, there is no biochemical diagnostic and prognostic methods to differentiate chronic venous ulcers of easy or difficult cicatrization, it was necessary to search for specific molecular markers through proteomic approach. The aim of this proposal is to identify the proteins present in inflammatory exudates of patients with chronic venous ulcers, in order to find specific molecular markers that can infer the stage of ulcer healing easy and difficult to heal, before and after the use of the new fibrin sealant CEVAP. This proposal has pretension to bridge the gap between discoveries at the bench and assistance patients at the bedside (from the beanch to the bedside). The future goals after the final analysis of the results will be: to suggest the insertion of proteomics analytical tools clinical routine in dermatology to have a precocious prognosis of ulcer healing; to transfer knowledge from basic research improving and creating new diagnostic methods to chronic venous ulcers; to establish therapeutic platforms capable of immediate clinical use, and, finally, to assist in the patenting of the new fibrin sealant, which is considered strategic and confidential at this moment.
