Clinical trial of low-level laser effect on pain, mandibular movement and psychosocial profiles reflection in different subgroups of TMD in the elderly

Abstract

The aim of the present study is to evaluate the prevalence of temporomandibular disorders (TMD) in elderly and assess the effect of low level laser therapy on pain, mandibular movement and its reflection in the various psychosocial sub-groups of TMD. The sample will consist of seniors evaluated on the Dental Clinic of a private institution of Higher Education (IES). All participants will be evaluated for the presence of TMD, its signs and symptoms as well as the sub-group of TMD, using the Axis I and II of the Research Diagnostic Criteria for Temporomandibular Disorders (RDC / TMD), formally translated to Portuguese. Among them, 20 individuals with TMD will be randomly selected for pain degree evaluation (Visual Analog Scale) and to receive the low level therapy. Individuals will be divided according to the sub-group of TMD. Patients with arthrogenous TMD will receive irradiation of diode laser, infra-red mode (ArGaAl) with 780·m of wavelength, with power of 70mW, 105J/cm2 density of energy during 60s. The laser will be applied in 5 points of the temporomandibular joint. In cases of myogenic TMD, the laser application will be made with 10 mW of power, 5J/cm2 density of energy during 20s at the point of pain related during the physical examination of muscle palpation. In both groups, the laser will be applied twice a week, totaling 8 consecutive sessions. Before starting the laser therapy, at the 4th session and at the end of treatment, patients will be assessed regarding the pain degree by visual analogue scale (VAS). At the end of the therapeutic protocol proposed, the mandibular movements and the psychosocial effects will be evaluated by axis I and II of the TMD/RDC. For the statistical analysis to check the distribution of the sample will be used the Kolmogorov-Smirnov test. The chi-square test will be used to evaluate the association between qualitative variables and the Spearman correlation to identify the relationship between the variables of outcome. (AU)
