Diagnostic characterization of secondary headaches to temporomandibular disorders in masticatory muscles: a controlled study

Abstract

Temporomandibular disorders (TMD), which are a group of disorders that affect the temporomandibular joint, masticatory muscles and associated structures, together with headaches, are very common painful conditions in the general population. As headache may occur in close temporal relation to the TMD is likely that may be a type of headache secondary to myogenic TMD. However, the current diagnostic criteria of the International Headache (IHS), do not include the headache that was possibly secondary to problems involving the masticatory muscles. Furthermore, the presence of pain sensitivity in pericranial muscles, including the temporalis and masseter is considered a subdivision of tension-type headache (TTH). But, these same muscles are also related to TMD. Therefore, this study aims to test the null hypothesis that headache reported by patients with TMD involving the masticatory muscles is not secondary to this condition, no disappearing within two months and not resorting after successful treatment of TMD. Thus it will be possible to assess the presence of headache which may be secondary to myogenic TMD and elaborate diagnostic criteria to define such painful disorder. For that, patients between 18 and 45 years will be evaluated for the presence facial pain complaints in the Orofacial Pain Clinic, School of Dentistry of Bauru, USP. Patients must be diagnosed with myofascial pain according to the Research Diagnostic Criteria (RDC / TMD). In addition, a questionnaire on headache of Headache Clinic Craniofacial Hospital of the Faculty of Medicine of Ribeirao Preto (USP-HCFMRP) will be applied to all patients. Thus, the sample will be comprised of 80 subjects and will be divided into: group I - patients with TMD and headache report, and group II - patients with TMD and no headache report. All patients will be treated for TMD in a minimally invasive way for 2 months periods and at the end of treatment there will be an assessment after 2 months and after 5 months of the beginning of the treatment. Statistical analysis of the results will be performed comparing and verifying possible relationships of measured variables in the assessments that will be: average of headache days per month, average days per month facial pain, average of pain intensity, average of pressure pain threshold (PPT), proportion of patients with headache remission in group I and proportion of patients with remission of facial pain. From the achieved results will be described diagnostic criteria for headache secondary to myogenic TMD. (AU)
