Abstract A varicocele is the main treatable cause of male infertility. Although present in 15% of the adult male population, 35% of infertile men and 80% of men with secondary infertility have a palpable varicocele. In addition, many studies associate varicocele to a decrease in semen quality, functional quality of spermatozoa, in testicular volume and proteomic composition of seminal plasma. The surgery (varicocelectomy) can improve sperm quality, but still do not know (i) which men should undergo surgery for varicocele and (ii) which molecular mechanisms determine this seminal improvement. A recent study from our group found that varicocelectomy leads to an early improvement (within 90 days) in seminal oxidative stress and in the biosynthesis of eicosanoids. We know therefore, that the correction of varicocele positively impacts the quality and functional expression of some proteins. We still need better map the proteome of seminal plasma and spermatozoa of these patients, because we do not know how it differs from semen of fertile men, neither can suggest which patients should or should not be operated.As a result, this project aims to study men with normal semen without varicocele (controls) and compare them with patients with varicocele before correction of varicocele and 90 and 360 days after subinguinal microsurgical varicocelectomy. In both groups (with and without varicocele) and in the three experimental periods (before, after 90 days and 360 days after varicocelectomy) will be performed an avaliation of sperm quality, functional sperm quality, the level of seminal lipid peroxidation and also a proteomic and lipidomic analysis of these patients.With these results, it is expected to get a better understanding of the mechanisms by which varicocelectomy can reestablish seminal homeostasis and the metabolic pathways present, as well as separate patients who will benefit in fact of the surgical intervention. | |
